CURRENT STUDENTS – ANNUAL REGISTRATION
Annual student registration is required for students grades K – 12. Registration forms should be completed by the primary parent/guardian by logging into the POWERSCHOOL PARENT PORTAL and clicking the Forms Icon. If you need assistance or your password reset, please contact your student’s building secretary or district registrar.
 
NEW STUDENTS – ENROLLMENT

STEP 1 | Fill out the Enrollment form:
          – K-12 Enrollment Form
          – 3 & 4 Year Old Preschool Enrollment Form

STEP 2 | PROOF OF RESIDENCY: All families must provide proof of residency, appropriate forms include:
If you own your home, please provide ONE of the following:
          – A copy of a recent mortgage statement
          – A copy of a utility bill for the current month (gas, water, electric, etc.)         

If you rent please provide ONE of the following:
          – A copy of your signed lease agreement with term listed. The agreement MUST contain property owner’s name, address and signature; and name and signature of parent/guardian
          – A copy of a utility bill for the current month (gas, water, electric, etc.)

If you live with a family already in the district, please bring ALL of the following:
          Completed and NOTARIZED RESIDENCY FORM and
          – Verification of residency for the family with whom you are residing (e.g., Utility bill, lease agreement, mortgage statement)

IMPORTANT |
         
– Post Office Boxes do not establish residency.
          – Falsification of any information or document required for residency verification or the use of the address of another person without actually residing there may result in:
          – revocation of the student enrollment;
          – being held liable to pay tuition for the time in attendance as a non-resident student;
          – and filing a complaint with the appropriate law enforcement agency for criminal prosecution against all parties involved.

STEP 3: ONLINE REGISTRATION FORMS
After you’ve completed the initial enrollment form and provided proof of residency, you will receive an email with instructions on how to create and login to the POWERSCHOOL PARENT PORTAL. Click the Form Icon under the left navigation menu to submit your student’s registration forms.

STEP 4 | REQUIRED DOCUMENTS
          -Proof of Age/Birth Certificate (PK-12)
          -Immunization Records (PK-12th)
          -Physical Form (PK & Kindergarten)
          -Vision Screening (Kindergarten & 3rd grade)
          -Dental Screening (Kindergarten & 9th grade)

Under Iowa Code section 282.3 a child MUST be five on or before Sept. 15 to enter kindergarten and must be six on or before Sept. 15 to enter first grade. Birth certificates or immunization records are examples of proof of age. Alternatives include an adoption record, certified statement of a physician, or anything else that is reasonably reliable.

STUDENT HEALTH REQUIREMENTS

Medical Documents: Doctor’s Office copy of your child’s Current Immunization Record or Exemption Certificate
Immunizations must be on file with the school nurse prior to the child starting school.
Iowa law requires a parent to verify proper immunization against diphtheria, tetanus, pertussis, polio, measles, mumps, rubella, hepatitis B and varicella to enroll in school. At least one dose of each immunization must be given before starting school. An immunization form may be provided by a child’s healthcare provider, it is also linked below.

Iowa Immunization Requirements: Iowa Code, Chapter 139a.8(6) and Iowa Administrative Code, 641-7.7(139) outline the immunization requirement for students attending licensed child care centers and elementary or secondary schools. Students shall have received the required immunizations and submit the Iowa Department of Health and Human Services Certificate of Immunization or have a valid Certificate of Immunization Exemption, or Provisional Certificate of Immunization.

The goal of the Iowa Department of Public Health, Immunization Program is to reduce and ultimately eliminate the incidence of vaccine preventable diseases by working in conjunction with public and private health care providers throughout the state. Accomplishing this goal will require achieving and maintaining high vaccination coverage levels, improving vaccination strategies among under vaccinated populations, prompt reporting and thorough investigation of suspected cases and rapid institution of disease control measures.

The Iowa Department of Public Health (IDPH), Bureau of Immunization, has completed the administrative rules process to require meningococcal (A, C, W, Y) vaccine for students enrolling in 7th and 12th grades. The administrative rule change was effective January 11, 2017, and will be implemented at the beginning of the upcoming 2017-2018 school year. Students entering preschool and kindergarten are required by Iowa law to have a physical examination by a licensed physician and provide proof of exam to the school district.

Required by Iowa law, dental screenings are to be completed for all students entering kindergarten and ninth grade. Students entering preschool and/or kindergarten must have had a screening completed no sooner than three years of age and no later than four months after school enrollment. It can be completed by a licensed doctor, advanced registered nurse practitioner, physician assistant, nurse, dental hygienist, or a dentist.  The results of this screening need to be submitted to your school nurse on the designated form linked to the below button.

The State of Iowa requires students entering kindergarten and third grade to have a vision screening prior to the first day of school.  This exam can be completed by a licensed eye doctor or optometrist, your child’s health care provider at the time of his or her school physical, a free clinic, or a county health department. A screening will be valid if it is done up to 1 year prior to enrollment and no later than six months after enrollment.  The results of this screening need to be submitted to your school nurse on the designated form linked to the below button.

Your child’s health conditions should be kept-up-to-date on school medical records. Health conditions such as ADHD, asthma, diabetes, heart problems, or seizures, just to name a few, should be on the school records.
